Host: Summer is almost here when we all live in lighter and few clothes and we’ll look at that problem fat that just won’t seem to go away no matter how much diet and exercise are involve but there is a fix for that fat, it’s liposuction and its come a long way and there are a number of lipo options out there and if you’re interested and here to sort it all out is Dr. Alberico Sessa from Sarasota Surgical Arts. Surgeon: Hi Heidi.
Host: What is liposuction?
Surgeon: Well, it’s exactly what it sounds like. Liposuction is where we infuse the body or the areas with fluid and then we remove it over the last 15 to 20 years there’s been a lot of changes in Liposuction, it’s become a lot safer, not that it ever wasn’t but the majority of Liposuction is actually can be done under just local anesthesia without the need for anesthesia at all without the need to go to sleep. In the past few years we’ve seen the occurrence of something called Laser Liposuction as well as Ultrasonic Liposuction. These are all techniques to help to shrink the skin and break up the fat so that it’s more easily removed without having to be a rigorous activity.
Host: Right. Ok, so laser, ultrasonic and also suction assisted, that the first kind, is that the….
Surgeon: Yeah, that’s the kind that has been around all the time. In order to remove the fat we actually have to take it out under vacuum pressure. We don’t just break it apart and then the body reabsorbs it to a certain extent that happens anyway. But most of it, especially in bulkier procedures it has to be suctioned out.
Host: Alright now, what about the effectiveness of it? Should anyone just go and have lipo, and then there’s the, is it gone forever or is it really a serious situation that you have to consider carefully?
Surgeon: Well, as always any surgical procedures have to consider very, very carefully but it is for just about everyone. We don’t recommend Liposuction as a weight loss. We recommend it the ideal patient would be somebody that is just looking to contour certain areas, maybe their stomach, maybe their thighs, maybe their love angles, that doesn’t go away with exercise or diet and in those patients; those are the ones that really see the greatest results. If it’s just for de-bulking tissue and to remove lots of large volumes, we really don’t recommend that and it sort of has, its not quite as popular as it was say 20 years ago or 25 years ago where large volume de-bulking procedures, it’s mostly Liposuction is sort of common into the buttock area, you know, where small areas bother people, I want you know, the curves of my hips to be more pronounced, I want my stomach to, I want to see the muscles in my stomach more. It’s not, you don’t have to be fat to have this done, you can have little localized areas and it’s great. And a lot of times we do these procedures just under just some simple Lidocaine which is just some numbing medicine.
Host: What about recovery, and how long this last?
Surgeon: You know, any type of surgical procedure is going to last for as long as you take care of yourself, but you know we would experience the normal post operative period of 2 to 5 days of some mild bruising some mild swelling depends on the areas that we treat but if it’s just localized small areas you may experience very little discomfort and very little post operative edema.
Host: I thought that was very interesting what you’re doing with some of the fat that you suction out and we’re not going to talk about that right now, I’m going to show you that at 6 o’clock. Alberico Sessa is an Italian American, born in Brooklyn, New York and raised on Staten Island. He graduated with a bachelor’s degree from Fordham University where he was a member of the Fordham University Men’s Crew. He received his first post-graduate degree from the University of Connecticut in 1997. Dr. Sessa earned his medical degree from the Louisiana State University College of Medicine in 2000. He then served his internship in General Surgery at Charity Hospital, and afterward completed his residency in Oral and Maxillofacial Surgery at the Louisiana State University, New Orleans, one of the nations top 2 programs.

So what are some alternatives to mastopexy or breast lifting?
Well, the first thing that we can discuss that sometimes an augmentation will be all you would need instead of going to scarring around the nipple a lot of people just don’t want that. So what’s an alternative is an augmentation. How do we know if it’s possible? If the nipple is at or above the fold and all we have is just some emptiness because the breast is deflated then the nipple isn’t gonna change but your breast will become more full and rounder from the implant so it’ll become artificially full instead of mammary glands being engorged so augmentation is a good way to avoid mastopexy in that instance.
The next way is very simple if it just slightly at the infirmary fold or just maybe slightly below the infirmary fold and you have what's called the grade one ptosis or very, very mild sagging of the nipple. Sometimes we can fix that with the combination of doing augmentation with something called the crescent lift.
Crescent lift is just very simply taking the skin that lies above the nipple and just removing a little wedge of skin so you end up with little hairline scar at the very top of your nipple and that will allow us to remove that little bit of skin, raise the nipple to a more natural appearance and then the augmentation will fill it all out better. And that's the nice way to maintain a more youthful breast with very, very little scarring.
