Recovering from a breast augmentation is not a difficult thing to do. You're never house bound, you're never bed ridden. People don't have to know you've had it done at all just based on what you wear.
So, there's a little bit of discomfort in the beginning because we put the implants under the muscle in everybody because your mammograms will be better, your scar tissue formation around the implants is less, they look better and they stay in a nice youthful position longer.
So, by going under the muscle the muscle has a little bit of a stretch from the implant and that causes a little discomfort but that's easily managed with medications and a little bit of time.

Hi, I'm Dr. Richard Rand. I'm a board certified plastic surgeon at the Northwest Center for Aesthetic Plastic Surgery in Bellevue, Washington. I'm going to talk with you about breast lifts.
It is a true art form. It's a sculpting surgical procedure. It's not like putting a breast implant in. A breast implant is a far simpler surgery to do. A breast lift is truly a sculpting of the breast.
A breast lift is a surgical procedure done for somebody who feels that their breasts have lost tone and shape. There's a condition called breast ptosis which is sometimes a droopiness of the breasts and when a patient has that, typically the nipple and the areola have gone down to the crease below the breast and they're either below the crease or pointing downward. And those people don't care for how their breasts sit on their chest wall. They don't feel youthful, they don't feel toned, they don't feel attractive.
So a breast lift is that surgical procedure meant to move the breast back up on the chest wall. To elevate the nipple above the crease below the breast. To tighten up the skin envelope of the lower pole of the breast. And to restore and aesthetic conical breast shape that looks youthful and attractive.
The occasional patient also needs more volume and that's where an implant comes in. An implant is the thing that adds size and fullness but the implant isn't the thing that lifts the breast. This is something that's very commonly misunderstood online. Patients want to believe that a breast implant can lift their breast up and elevate the nipple. That doesn't happen. But a breast lift is the thing that you need if the nipple and areola are sitting too low and too lax or below the crease below the breast.
There's a variety of scar patterns that are used depending on surgeon's preference and depending on the patient's anatomy. And that's also important for people to really carefully look at one surgeon's scars vs. another. Because a breast lift is a trade off of shape for scar. So you want, if you're going to get this done, to have not only good scars but a great shape.
Fortunately, one of the surprising things for patients who have a breast lift is that it's one of the most painless surgeries that we do. People who are not having muscles stretched or muscles tightened usually don't have that much discomfort. So, while each person is unique we have them exercising at three weeks and doing virtually everything by two months.

They tell me they love the result. They tell me they feel restored they feel attractive. They love the flexibility in their clothing choices. They feel that they can present themselves in the world in a very confident fashion.
Breast augmentation is typically performed on people who've had children, who've lost breast volume and lost breast tone. And who want to restore their youthful body. Occasionally people will simply just not develop as much breast tissue as they wished they would and those people come in also seek breast augmentation consultation.
Breast augmentation is my most commonly performed surgical procedure. I do between 100-150 of them every year. And it is one which is very safe. It takes about 40 minutes to do. And patients recover from it rapidly.
I typically use the incision in the inframammary crease because that incision has been shown by recent scientific studies to be associated with the least amount of scar tissue forming around the implant. The so-called capsular contracture which is the result we don't want people to have.
Recovering from a breast augmentation is not a difficult thing to do. You're never housebound. You're never bed ridden. People don't have to know you've had it done at all just based on what you wear. So there's a little bit of discomfort in the beginning because we put the implants under the muscle in everybody because your mammograms will be better. Your scar tissue formation around the implants is less. They look better and they stay in a nice youthful position longer. So by going under the muscle, the muscle has a little bit of a stretch from the implant and that causes a little discomfort. But that's easily managed with medications and a little bit of time.
National averages for breast augmentation revision can be in the 20 percent range or more. Those are very high numbers and we find that unacceptable. So in our office, and my patient population, we revise at most one person a year. We really try to get it right the first time because we don't want people to go through revisions for implant displacement, implant size issues. We want to get it right once.

Face lift surgery requires an ability to create inconspicuous scars around the ears. Dr. Rand uses a careful design and technique, a post-tragal approach in women, incision closure under no tension, and a meticulous eye regarding the aesthetic units of the area.
Dr. Rand believes that a multi-plane technique such as a SMAS lift gives the best results without looking operated upon.

Hi, I'm Dr. Richard Rand. I'm a board certified plastic surgeon in Bellevue, Washington about 5 miles from Seattle. Today, I'm going to talk to you about lower facelift surgery or sometimes called cheek and neck-lift surgery.
The facelift patients are my very happiest patients. They love the result. The love the quick recovery, doesn't have much pain. They love how refreshed and rejuvenated they look and they love the fact that they don't look operated on.
Patients considering having a a lower facelift or cheek and neck lift, they're beginning to see signs of aging. Gravitational descent of the cheeks, jowls along the jaw line, laxity in the neck that make them feel as though their face doesn't reflect the youthfulness they've striven so hard to achieve.
These surgeries are done in order to elevate the cheeks back up to their youthful position. Eliminate the jowls along the jawline so they look nice and smooth and youthful. And develop a nice angular neck which doesn't have loose skin or extra fat or loose muscles anymore.
The scars from a lower facelift or cheek and neck lift are very carefully crafted around the ears, in front of the ears, behind the ears in a way where only your hairdresser might know they're there. There's also a scar under the chin that allows for the proper access to the front of the neck so the front of the neck is completely rejuvenated. Between those incisions the entire surgery can be done. So there really aren't any visible scars. You have to really know where to look to find them.
In my practice, the way I perform the surgery, about 95% of the patients look extremely presentable in social situations at the two-week point. The final 5% may take three weeks but never longer than three. So 95% would be able to feel visible in public at two weeks.
I think that facelift surgery is the highest art form of aesthetic plastic surgery, after all this is your face. You can't hide a result that isn't perfect. For me I take great pride in the technique that I've worked on and developed over 25 years and would encourage people to look at all the pictures in the photo gallery and the website and come in and seek a consultation with me if they'd like to see if they'd be a good candidate for that surgery. It's my very favorite surgery to do.

People often ask how long a cheek and neck lift is going to last and whether they'll need revisions. And in my practice, early revisions are practically unheard of and most people would wait at least 10 years before they would even consider redoing it. And in reality most of them do it on a one-time basis for the rest of their life.
So the revision rate and the secondary surgery rate are almost zero.
